Orion Health's eSAP solution allows GPs and other health and social care to enroll elderly service users on a Single Assessment Protocol (SAP). The solution tracks their care using a web-based electronic health record. Orion Health's Clinical Workflow Engine creates an electronic version of the SAP clinical pathway and assessment forms. The eSAP care pathway provides a social services assessment of the client and their carers. Health providers such as district nurses and GPs can add clinical notes in the form of free text to the assessment too. The eSAP clinical pathway sits inside Orion Health's Concerto Portal, giving authorized social service and health care workers secure remote web-based access to patient data. Automatically generated tasks and reminder messages help ensure that all necessary assessments tasks are carried out. Each assessment has an associated form with a customised screen for collecting the required information, based on well-known SAP tools. The electronic forms are designed to be easy to navigate and quick to complete. Radio buttons and drop-down lists ensure that the forms are easy to complete while maintaining high data quality by restricting users to a choice of appropriate values.
Pre-populated information for ease of useUsing traditional paper-based systems, the onus is on patients to recall clinical information each time a new assessment form is created. Clinicians often handwrite answers on the form, and then type the information into their computer system, effectively duplicating their work. Orion Health's solution benefits users by allowing them to type information directly into forms without inhibiting their routine workflow.The solution also reduces the data entry burden by pre-populating information for them when the form is first displayed. Around the clock access to real-time dataThrough the creation of electronic records and workflow, GPs, district nurses and other authorised healthcare professionals have access to electronic health records 24 x 7, giving them real-time data they can trust. Informed decision makingThe eSAP web-based record supports a move from previous "treat as seen" methods to treatment that is based on knowledge of health events and that is in line with principles highlighted in the National Service frameworks. It enables better decision making from healthcare professionals, as it is person-centred and gives them the wider picture of Service User history and progress.Secure User AccessUsers access the system via a web portal front end server which resides on a secure network between the local council, the PCT and other agencies. Before access is given to any patient information, users must be authenticated using their unique username and password. The system restricts access based on the user's role and patient consent, displaying only the menus and screens that users have permission to view. |
