A unified view of a patient’s problem data

Documenting and reviewing a patient’s Problem List is a key way for clinicians to understand a patient’s health picture. To be an effective clinical tool, the Problem List must be accurate, complete and timely. Problem List provides a centralized, authoritative list of patient problems that is accessed via a familiar web interface. Problem List is the central place for authorized care providers to obtain a concise view of all patient’s medical problems whether they were manually entered by the provider, documented in an electronic medical record system, or even integrated from the patient’s Personal Health Record. Problem List unifies data into one simple view.

Problem List provides the ability to break through the various silos of information, whether it be an inpatients HIS, a specialist’s EMR, a patients PHR or a Physician’s notes in a patients chart. With Problem List problem data can be collected from external systems using industry standard message formats or it can be manually entered and updated using the Problem List user interface. Either way, Problem List provides a centralized view of information. In order to properly document and collect patient problem data, other systems need to be able to communicate.

Problem List uses standardized vocabulary to ensure the highest level of structured data possible. The following is a list of industry standards Problem List works with to ensure compliance with functionality and interoperability requirements:

  • Interoperability using industry standards: HL7, CCR, CCD
