Coordination and Sharing of Patient Care - Across the Community

cWorkflow Engine can coordinate tasks for mobile health workers and primary care agencies providing services to patients living in the community.

cWorkflow Engine supports a "patient-centric" approach to care, where all tasks and assessment data associated with a particular patient can be planned and shared between a multidisciplinary team of providers.

Depending on the requirements of the community care program, cWorkflow Engine can be configured to generate a wide variety of assessment forms, automated reminders and care tasks that reflect the work practices of the care team.

Setting care aims and measuring outcomes

cWorkflow Engine can allow health workers to record the objectives of a phase of care, (eg. for home-based physiotherapy / rehabilitation) and then record a patient's progress against these aims.

Integration of health and social services

cWorkflow Engine provides a platform through which health and social service agencies can share information. Assessment data collected by health workers can be automatically shared with appropriate social services agencies, reducing the need for repeat questioning of patients and providing all service agencies with a more complete picture of patient status. Equally, relevant electronic social services data can be integrated into the patient's electronic health record.

Prepopulation of data

Data that already exists in other information systems, as well as data previously entered by other care providers during the care program, can be pre-populated into the assessment forms. This reduces the burden of data entry on staff working in the community.

Sharing information and care objectives

Using cWorkflow Engine, care providers can view the completed and planned care interventions of their colleagues. This allows providers to adjust and improve their own interventions to better coordinate with the activities of colleagues.

Strict security and patient privacy

Via Concerto Physician Portal, strict patient privacy and security rules are maintained to maximize the confidentiality of patient information

Support for program-wide analysis and management reports

Program managers and team leaders can use cWorkflow Engine to track trends across a community-based patient population, allowing them to identify the interventions that are most effective, as well as highlighting areas where care can be improved.
