Patient Record
All recorded information regarding a patient’s clinical history, examination findings, diagnosis, treatment, and consent.  Also includes billing records associated with and documenting patient care.

Under data protection legislation and the law generally responsibility for patient records is always on the creator and custodian of the record, usually a health care practice or facility. The physical medical records are the property of the medical provider (or facility) that prepares them. This includes films and tracings from diagnostic imaging procedures such as X-ray, CT, PET, MRI, ultrasound, etc. The patient, however, according to HIPAA, owns the information contained within the record and has a right to view the originals, and to obtain copies under law.

Health Level Seven XML Patient Record Architecture – clinical documents are defined as legally authenticated (attested or signed) and persistent entries into a patient record. The architecture specifies a document markup format which enables documents to be transferred on-line or stored in files on off-line media. The HL7 Document is intended to be the basic unit of a document-oriented Electronic Patient Record (EPR). In the document-oriented patient record, whether computer- or paper-based, the patient's medical record is represented as a collection of documents.
